Neuromuscular Modulators/”Wrinkle Relaxers”
These injectable medications can be thought of as “wrinkle relaxers”. In the United States, the FDA-approved medications in this category are Botox, Dysport, and Xeomin. The products all work in a similar manner. They are injected into specific muscles of the face that lie under the facial skin. The muscles normally contract repeatedly every day, and contribute to facial wrinkles over time. But, by allowing certain muscle areas to relax, these products can be used to smooth out frown lines between the eyebrows, furrow lines on the forehead, and crow’s feet around the eyes, giving a more youthful, refreshed appearance.
The injection procedure is quick (typically under 15 minutes), virtually painless, and requires no recovery time (although it is advised that patients refrain from exercising for the remainder of the day). Results can be seen from three to seven days after the injection, and the effects typically last from three to four months. Because these injections produce a noticeable improvement in the look of moderate to severe wrinkles so easily and conveniently, they are very popular among patients seeking a non-surgical approach to facial rejuvenation.
A dermal filler (such as Restylane, Juvederm, Radiesse, and Belotero) is another type of injectable that is a minimally-invasive alternative for facial enhancement. Dermal fillers have the consistency of a soft gel, and work by various mechanisms. Some of these products contain hyaluronic acid (HA), which is a substance that occurs naturally in the skin and connective tissues. HA, (along with collagen and elastin, which form the support structure of the skin), is gradually lost over time, as part of the aging process. HA-based dermal fillers replenish the skin’s supply of this substance, and also draw in water, resulting in more youthful-looking skin. These fillers can be used to soften fine lines around the lips, fill-in the nasolabial folds (lines extending from the edge of the nose to the corners or the mouth) and marionette lines (extending from the corners of the mouth to the chin/jowel region). They can also be used to rejuvenate volume loss in the lips, cheeks, and other areas. Other types of dermal fillers are also available, and can be used to fill in deeper facial wrinkles or folds, replace volume loss, and achieve a more youthful contour of the jawline. This type of dermal filler contains tiny spheres of calcium hydroxylapatite, which is also naturally found in the body. It works by improving the structure of the skin from the inside, by stimulating the body’s production of collagen. Once injected under the skin, collagen forms around each sphere, increasing the volume of the targeted area. This method improves the skin’s overall strength and structure, while giving the skin a more smooth and supple appearance.
There are many FDA-approved dermal fillers on the market, and the choice of filler will depend on the area that is being treated and the desired result. Most patients report only minimal discomfort during the injection of dermal fillers, and additional numbing medication or a small ice pack can be used to reduce any potential discomfort even further. The most common side effects are some bruising near the injection site (which usually subsides within a week and can be concealed with makeup) and localized swelling (which generally resolves in 1-2 days). As with any skin injection, there is a risk of infection.
Dermal fillers can last anywhere from 6 to 18 months, depending on the type of filler being used and the area in which it is injected. Several different brands of fillers are available, including Restylane, Restylane Lyft, Restylane Silk, Belatero, Radiesse, and Juvederm Voluma products.
